LIFETIME BENEFIT MAXIMUM 

As per Healthcare Reform 2011, the plans have been amended to comply with this  provision. The Annual and Lifetime Benefit  Maximum has been eliminated.

 

DEPENDENT CHILDREN

Children remain eligible for coverage to age 30. 

NOW THEY DO NOT HAVE TO:
Live at home, be full time students, be unmarried, or have no dependents of their own.

 

Preventive Care $0 Co-pay
Co-pay for preventive care was eliminated in our plans to encourage members and their families to have their annual check-ups with age -appropriate screenings. (Check your health plan's website for information about recommended screenings).

No changes for 2015 in service provided. Mental Health Behavioral   benefits are being administered by CIGNA Behavioral Healthcare. The premium for 2015 will increase by 9.98%. Increase in 2014 7.47%. In 2013 5.74%; in 2012 6.4%, in 2011 was 7.5%. In 2009 8.5% and 10.5% in 2008. No changes for 2015 in service provided. Mental Health Behavioral   benefits are being administered by CIGNA Behavioral Healthcare. The premium for 2015 will increase by 9.97, In 2014 9.88%. The premium will increase 10.02%, In 2014 7.48; in 2013, 5.74%, in 2012, A decrease in 31.3% in 2011; in 2010 increased 7%. This plan was offered for the first time in 2009. Anthem (Empire) Blue Cross Blue Shield High Deductible Health Plan /HSA No changes for 2015 in service provided.  The premiun will increase 8.47%. In 2014 5.99; 4.16 % in 2013, 5.0% in 2012. This plan was offered for the first time in 2011.

Medicare Secondary Payer (MSP)
Employees who reach age 65 and work for a Small Employer (parish/organization with 19 or less employees) are entitled to use Medicare Part A for hospitalization charges and use their regular medical plan for Pharmacy and other medical costs. The only medical plans that has accepted to be linked to the MSP is: Empire High Opt MS PPO and EPO 100 Network only Your employer has to submit an Eligibility Certification Form and Small Employer Exception Form.
